REQUEST FOR SAFETY REPORTS

Things have really slowed down as far as operators requesting safety analysis. From March 15 there was not one submitted until July 28th!
Not surprising of course since so many of them have been disregarded. I personally have submitted four and have not had one adopted. Sounds kinda like the old yellow card system, but with a new name this time.



Request:
Not enough space between
stops. Wants stop moved

Answer:
Field Operations measured the distance and found that it is 581
feet from the beginning of the turn from the Transit Center down
to the stop. There is not a problem as buses are doing 15 MPH to
that point. Plus there is no traffic approaching from the rear or
right as eastbound traffic is held by the light before the Transit
Center. There is plenty of time and distance to service the stop
with room to spare

Request:
Unsafe turn/lane change. 90th/Sand

Answer:
TriMet planners have looked at this location and agree that it
could be improved. A resolution is in progress to correct this
issue. A split phase signal is being studied and if this is not a
possible option, then a zone and stop adjustment will be made.
Regardless of resolution, a change will be in place by the Oct 1,

Request:
Mis-positioned seat; too far
forward
Answer:
This issue was reported for this and one other bus. This has been
checked on both buses and both operator seats go all the way
back, as far as possible. The Maintenance Supv. also indicated
that the operator had a seat fail under him and break down.
Maintenance is ordering a new operator seat that has a higher
weight rating, and will want the operator to test it.

Request:Unsafe for sight impaired to cross MAX tracks Gateway
Answer:
We are in the process of evaluating all crossings in the rail system
and determining which safety treatments are needed. Gateway
will be a part of that evaluation. The treatments will be
prioritized and installed over time as funds come availabl

Request: Unsafe lane change/turn NW 6th/Glisan
Answer:No change. Field Ops. Measured the distance from NW 6th to
Broadway from crosswalk line to outer crosswalk line = 219.5
feet. Line 9 buses, after crossing the intersection, beginning with
the crosswalk line can count off 100 ft. This leaves 119.5 ft. to
merge into the left lane then turn left onto Broadway. Before
merging into the left lane on Glisan, the operator may have to
stop and wait for an opening so that he does not run out of room
to merge left before turning left onto Broadway

Request:
Police cars driving
dangerously around buses on
the downtown mall
Answer:
Request Operator contact Dispatch if this happens again and ask
that Safety be paged. Information must be collected regarding
specific police cars as event happens and the bus video tape can
be pulled

Request: Request to move unsafe stop.
N.E. Prescott &
82nd Ave
Answer:
This stop used to be on the farside of 82nd Avenue, but because
of a fatal accident, it was moved back to the nearside. More
passengers deboard and want to go south on 82nd than north

Request:
Brake Pedal comes off 2900
series bus bridging between
brake and accelerator
Answer:
3/21/11 This was an issue and the manufacturer was consulted.
The fleet was campaigned, twice; the first time, rubber cement
was used to secure the covers, but was not effective. During the
second campaign, clips were used to secure the cover. This work
was completed approximately 3 weeks ago and we believe that
the problem is resolved. If you see this issue again, please
immediately report it

Request:
Drivers can't see the
speedometer over the
steering wheel on 2900
buses
Answer:
Operator is correct that the steering wheel can block the
speedometer. However, with proper steering wheel adjustment,
and by leaning slightly in the seat, most operators should be able
to see the speedometer. Sent a document to operator which
provides additional information that may be helpful. Operator is
also welcome to contact the training department or garage
trainer for assistance with steering wheel adjustment or to
discuss this further
