Incident: Operator slipping on wet floor
At approximately 16:26, Operator entered the break room at the Tigard Transit Center. Mr. stated that when he entered the room, the floor was wet and his feet went out from under him and he landed on the floor. I was notified as I was the district Supervisor (). The Operator called dispatch by phone and informed them that he was on his back on the floor and wasn't going to move until I arrived there. The Operator informed Dispatch that he didn't need medical. I informed Dispatch that regaurdless of what the Operator stated, I still wanted him checked out. Because this is a locked facility medical personnel can not access this room. I had Dispatch notify the next bus into Tigard TC to open the door and remain there until medical personnel arrived. I was at 170th and Baseline and took off to Tigard TC as safely as possible. hearing this also sent () from downtown.
arrived shortly before myself. The ambulance (Metro West) was just leaving as was arriving. I found the Operator upright and walking slow when I arrived. I asked him what happened and he informed me that he walked in with his water bottle(open) in his hand and his backpack and the next thing he knew he was on the floor. I asked the Operator where he was hurt and he pointed to the same area on my back which was about 2/3rds down on his left side. I asked him if he thought he would be able to drive the bus back to the garage and he stated "yes". Mr. then walked over the bus and then walked back. When I noticed him walking back I asked him what was up. Mr. stated to me that the more he walked the more his back went "clicky click" and that he didn't feel he could drive the bus back. I immediately notified Dispatch of this. I had them call Mr. a taxi and to have him transported to Merlo Garage. I also informed Mr. to call Dispatch when he arrived.
When I first arrived both and Mr. was inside the break area. Both myself and was trying to figure out where the water, or liquid, could of originated from. Both and myself flushed both toilets and they both operated as intended. There was also no observation of any liquid on the floors of both the bathrooms. The floor of the hallway was very wet. made some signs that was placed on the door to use caution as the floor was wet. A call was also dispatched to all the busses using Tigard TC of the hazard.
